Certificate of Assumed or Fictitious Name
- Effective January 1, 2020, Certificate of Assumed or Fictitious Name forms will be filed solely with the State Corporation Commission. Only a released Certificate of Assumed or Fictitious Name form will be filed with Circuit Court and only if the original Certificate was put to record with the court.
